MEMBER WAIVER AND RELEASE OF LIABILITY

I {name} hereby acknowledge and agree to the terms outlined in this Waiver and Release of Liability in consideration of being allowed to participate in activities and use the facilities provided by Ultimate Shred Academy. I understand that engaging in physical exercise and fitness activities involves inherent risks, including the risk of serious injury or even death. By signing this Waiver, I voluntarily assume all risks associated with my participation in the Gym's activities and use of its facilities.

1. Assumption of Risk: I acknowledge that the activities offered by the Gym, including but not limited to weightlifting, cardiovascular exercise, group classes, and the use of gym equipment, can be physically demanding and may expose me to certain inherent risks. I am aware that there is a risk of injury, illness, or other harm associated with physical exercise, and I willingly assume all such risks.

2. Health and Medical Condition: I certify that I am in good physical condition and have no medical condition that would prevent me from participating in physical exercise and fitness activities. I understand that it is my responsibility to consult with a medical professional prior to starting any exercise program to ensure it is safe for me to do so.

3. Release of Liability: In consideration of being permitted to participate in the Gym's activities and use its facilities, I, on behalf of myself, my heirs, assigns, and legal representatives, hereby release, discharge, and hold harmless Ultimate Shred Academy, its owners, employees, agents, contractors, and affiliates, from any and all claims, liabilities, damages, actions, or causes of action arising out of or in any way related to my participation in the activities provided by the Gym, including but not limited to any negligence on the part of the Gym or its staff.

4. Indemnification: I agree to indemnify and hold harmless Ultimate Shred Academy, its owners, employees, agents, contractors, and affiliates, from any and all claims, demands, liabilities, suits, actions, or expenses arising from or in any way connected with my participation in the Gym's activities or use of its facilities.

5. Photography and Likeness: I consent to the use of any photographs, video recordings, or other likenesses taken of me during my time at the Gym for promotional purposes without compensation or notice.

6. Governing Law: This Waiver shall be governed by and construed in accordance with the laws of [State/Country], and any legal action arising out of or related to this Waiver shall be brought in the appropriate courts of [State/Country].

I {name} acknowledge that I have read and understand the terms of this Waiver and that I am signing it voluntarily. I am aware that by signing this Waiver, I am giving up certain legal rights. I further acknowledge that I have had the opportunity to seek legal counsel before signing this Waiver.
